Patients will be asked to fill out and sign THREE FORMS, a patient screening form, waiver form, and medical history form. Forms will need to be filled out and emailed back to the office no later than 24 hours before your appointment. You are able to download and fill out forms through both your computer and cellphone.

*INSTRUCTIONS FOR DOWNLOADING FORMS THROUGH COMPUTER*

1. Open the file and download it to your desktop
2. Find the file on the desktop and right-click on the file
3. Click on option Edit with Adobe Acrobat
4. A pop-up message will emerge, click on the edit form fields option and continue
5. Fill out the form and save it on the desktop. Attach the form to an email and send the forms back to us. Repeat steps for both forms.

Please arrive 5 minutes before your appointment. When you arrive, please wait in your car and call the office at (408) 739-9047 or text us at (408) 475-6751 to check in and let us know you are here. We will call you when we are ready to see you.

Dr Cano’s office and SCCDS strongly recommend face masks to be worn in the dental office.

We request that ONLY the patient come into the office. ONLY patients are allowed into the treatment area. We no longer have a waiting room.

Patients will have their temperature taken and asked another set of questions before entering the office.

Please be on time….late arrivals will NOT be accommodated.
